Disclaimer and Waiver of Liability

I acknowledge that I have voluntarily chosen to participate in fitness activities from The Agile Athlete, LLC. These include, but are not limited to strength training, flexibility exercises, aerobic exercises, and the use of exercise equipment and machinery. I understand the nature and potential intensity of the programs offered and am aware of the inherent risks involved.

Assumption of Risk I willingly assume full responsibility for any and all risks associated with my participation in the fitness program. I understand that, while unlikely, my participation may result in physical harm, including but not limited to muscular, skeletal, or nerve damage.

Health Representation I warrant that I am in good health and have no medical conditions that would increase my risk of injury during the fitness services. I declare to be free from any condition, impairment, disease, infirmity, or other illness that would affect nutrient metabolism or impede safe participation. I have not informed The Agile Athlete, LLC of any reasons why I may be unfit to receive their services. I confirm I have received medical clearance for participation or elect to participate without physician approval, assuming all responsibility for such a decision.

Release of Liability  In consideration for being permitted to participate in The Agile Athlete, LLC activities and use its programs and equipment, I hereby release and forever discharge The Agile Athlete, LLC,  its owners, members, managers, officers, principals, independent contractors, and employees from any and all liability, from any and all claims, demands, rights, and causes of action of any kind, arising from or by reason of any personal injury, property damage, or the consequences thereof, resulting from or in any way connected with my participation in any activities or use of any programs and equipment.

I understand that by signing this document, I am voluntarily waiving my right to make a claim or file a lawsuit against The Agile Athlete, LLC, even if they negligently or by some other act or omission cause injury or damage.

Future Visits This waiver remains in effect for future visits unless revoked in writing.

I have read and understand this disclaimer and waiver in its entirety. By signing below, I acknowledge that I have voluntarily assumed the risks associated with the fitness program and have released The Agile Athlete, LLC from all liability.
